Inpatient admission
If you are a GP or healthcare provider and you feel that your patient or client needs to come directly into hospital, you will need to send an eReferral electronically via Healthlink or via your GP Practice IT Management System.
Alternatively, you can download and complete a either the adolescent or adult referral form and fax the completed form to the Referral and Assessment Service at 01 249 3609.
When the assessment team receives this referral form, your patient or client will be contacted by a member of the assessment team to arrange admission.

Dean Clinic (outpatient service)
For less urgent cases, you can refer your client for a mental health assessment at our community mental health clinics, called the Dean Clinics.
- eReferrals can be sent electronically via Healthlink or your GP Practice IT Management System
- You can also complete a either the adolescent or adult referral form and return it either via fax: 01 249 3609 or via post to Dean Clinic Referrals, St Patrick’s University Hospital, James’ Street, Dublin 8.

When the Dean Clinic team receives this referral form, both you and your client will receive confirmation of appointment at the clinic.
Day services
If you wish to refer your client to our day service, you need to complete a Referral form and return it either via fax (01 249 3609) or via post to the Referral and Assessment Service, St Patrick’s University Hospital, James’ Street, Dublin 8 where they will be assessed and referred to the appropriate programme.
