Help improve mental healthcare
St Patrick’s Mental Health Services (SPMHS) is taking part in the National Mental Health Experience Survey.
Service users who recently left our inpatient mental health service may now be able to complete the survey.
This survey asks people about the mental healthcare they received in hospital. The results will help improve services for others.
The survey is developed by the National Care Experience Programme, a collaboration of the Health Service Executive, the Department of Health, HIQA (the Health Information and Quality Authority) and the Mental Health Commission.
Why we are asking for your consent
In order to be able to invite you to take part in the survey, we need your consent to share your name and postal address with the national survey team.
The survey team will then contact you shortly with information about the survey and how to complete it.
You will find a statement of consent and a consent form to take part in the national survey further below.
What happens if I say yes?
If you give consent:
- we will share your name and postal address only with the national survey team
- the team will contact you with information about the survey
- you can decide then if you want to take part.
What happens if I say no?
If you do not give consent:
- we will not share any of your information
- you will not receive an invitation to take part
- this will not affect any care or support you receive.

Statement of consent
I hereby consent to St Patrick’s Mental Health Services providing details of my full name and postal address to the National Mental Health Experience Survey team (operated by the National Care Experience Programme) for the purpose of enabling them to contact me in relation to their planned national survey of mental health inpatient experiences.
I understand that I may revoke this consent at any time before my information is transferred to the National Mental Health Experience Survey Team, by emailing nationalsurvey@stpatricks.ie or by writing to National Mental Health Experience Survey, St Patrick’s Mental Health Services, James’ Street, Dublin 8.
I understand that if my information has already been released based on this consent, my withdrawal will not apply to information already shared.
